ABOUT DR. DEANTONIO
Michele DeAntonio, MD, FAAP, is a Board-Certified Pediatrician with years of experience treating patients for Lyme Disease and related tick-borne illnesses, PANS/PANDAS and other complex medical conditions. Dr. DeAntonio is a member of the Lyme Disease Association, the PANDAS Physicians Network, and the Institute for Functional Medicine. She earned her Medical Degree at the University of Pittsburgh School of Medicine, and completed her residency at the University of Miami's Jackson Memorial Hospital, where she served as Assistant Chief Resident.
ABOUT DR. DEANTONIO'S PRACTICE
Dr, DeAntonio was one of the first pediatricians to see the threat posed by Lyme Disease and get the education necessary to effectively diagnose and treat tick-borne infections. The same holds true for the neurological & physical complications arising from strep & other infections known as PANS/PANDAS. She is tireless in researching the latest scientific developments, and promoting patient education. Dr. DeAntonio is devoted to providing her patients with compassionate care, utilizing evidence-based medicine. Genetic testing, nutrient & environmental toxin analysis,& food allergy & sensitivity testing are part of a comprehensive evaluation to achieve optimum health. Dr. deAntonio's approach combines the use of nutritional supplements and botanicals, plus antibiotics, antivirals, and anti-parasitics as warranted.
OFFICE POLICIES & INFORMATION
1) MICHELE DEANTONIO, MD DOES NOT ACCEPT INSURANCE.
Patients are responsible for payment in full at the time of service. New patients are required to make a non-refundable deposit of $500 to schedule the first appointment. Receipts will be provided to patients, but Michele DeAntonio, MD assumes no responsibility for any issues regarding insurance coverage or reimbursements to patients.
2) 24 HOUR CANCELLATION POLICY
All appointments must be cancelled or rescheduled no later than 24 hours in advance, except Monday appointments, which must be cancelled/rescheduled the prior Friday. ALL CANCELLATIONS MUST BE MADE BY PHONE TO OUR STAFF DURING REGULAR OFFICE HOURS. EMAILS & PHONE MESSAGES ARE NOT ACCEPTABLE.
3) PATIENT AGREES TO PAY A NON-REFUNDABLE DEPOSIT OF $500 FOR INITIAL APPOINTMENT.
Fee must be paid when appointment is made, and will be applied to the total balance due from your office visit or consult.
